By applying for membership in the Universal Palm Tree Woman (UPTW), I/we make the following statements and agree to abide by the following conditions: I/we will at all times abide by the Bylaws, Policies, Procedures, Code of Ethics, Rules and Regulations of UPTW; I/we accept and understand that the amount of my monthly dues determines the type of support and services I/we will receive. I understand that this membership is for 10 members per family. With this membership application, I will pay the initial enrollment fee and 12-month dues. I agree that UPTW may release my contact information to affiliated clubs, UPTW members, UPTW sponsors, and the general public.
